While it is true that female fertility declines more rapidly with age, I have seen many couples where the male partner’s fertility played an equally important role in their difficulty conceiving. I remember one gentleman in his early forties who was convinced everything would be normal because he felt healthy and his semen analysis from years ago had been “fine.” His sperm count was still within the normal range, but further testing showed increased DNA fragmentation, which explained why the couple had experienced repeated failed IVF attempts.

One of the questions I hear most often is, “Does sperm count decrease with age?” The answer is that it can, but the decline is usually gradual. Some men in their forties and even fifties still have sperm counts within the normal range.

However, fertility is about much more than the number of sperm present. Healthy conception requires sperm that swim efficiently, have a normal structure, and carry intact genetic material. This is why I never look at sperm count in isolation. A comprehensive evaluation always tells us much more.

I have treated many couples where the male partner was in his forties or older and they successfully achieved pregnancy.

One misconception I frequently encounter is that IVF or ICSI completely overcomes age-related male fertility issues. These advanced treatments certainly help us address many male factor infertility problems, especially when sperm count or motility is reduced. However, they cannot entirely eliminate the effects of poor sperm quality or increased DNA damage.

Research has shown that advancing paternal age may influence:

  • Fertilisation rates
  • Embryo quality
  • Implantation success
  • Risk of miscarriage
  • Overall pregnancy outcomes in some couples

Whenever I evaluate a couple, I always assess both partners together because successful treatment depends on understanding the complete picture rather than focusing on one individual.

What Men Can Do Regardless of Age

One of the most encouraging parts of counselling couples is explaining that there are several steps

men can take to support their fertility. While we cannot stop biological ageing, we can improve the environment in which healthy sperm develop.

I usually recommend:

  • Maintaining a healthy body weight
  • Exercising regularly without overtraining
  • Getting adequate sleep
  • Managing stress effectively
  • Avoiding smoking and limiting alcohol
  • Controlling diabetes, blood pressure, and other chronic illnesses
  • Avoiding excessive heat exposure to the testes
  • Eating a balanced diet rich in antioxidants

Nutrition deserves special attention because antioxidants help reduce oxidative stress, which can contribute to sperm damage. I always remind my patients that sperm production takes around three months. Lifestyle improvements today may begin to show benefits after several weeks, so consistency matters.

When Should an Older Male Partner Get Evaluated?

I believe this is one of the most important questions couples should ask. Many people assume fertility testing should begin only after multiple failed attempts at conception. In reality, earlier evaluation often saves valuable time.

I generally recommend a male fertility assessment if:

  • The male partner is over 40 years of age
  • Pregnancy has not occurred after six to twelve months of trying
  • There is a history of recurrent miscarriages
  • Previous IVF or ICSI cycles have been unsuccessful
  • There is a history of testicular surgery, chemotherapy, infections, or significant medical illness
  • There are concerns about sexual function or hormonal health

A comprehensive evaluation may include semen analysis, hormone testing, physical examination, and additional investigations such as DNA fragmentation testing when clinically indicated.
